Medical Declaration
As per Department requirements, parents/guardians have to submit a Return-to-School Medical Declaration for their child(ren). This needs to be completed each time your child is absent from school.
To do so, please click the link below. You enter the school’s roll number which is 19702N and click Submit. Then enter your own name, the name(s) of your child(ren) and the class they are in. Then click Submit Declaration.
